Application notes for Correction of personal details form

How to apply

  • Complete the Correction of personal details form.
  • Documents not in English must be translated — see Translation
  • All documentation must be stamped and certified by a Justice of the Peace or Commissioner of Declaration.
    The registration number and full name of the certifier must be included and be legible.
  • Check information recorded is accurate before you submit your application.

Evidence of identity

The QCAA must verify your identity before we can amend your personal details.

Identification requirements are to minimise the potential for fraud and to ensure the privacy of your personal information.

Acceptable forms of identification

Evidence for change of personal details

  • Birth certificate
  • Linking birth certificate
  • Change of name certificate
  • Other - you will need to provide additional documentation.

Translation

Documents produced in a language other than English must include a written translation in English by an accredited translator. To find a translator, go to the National Accreditation Authority for Translators and Interpreters (NAATI) website.

You must include a certified copy of the original and the translation.

Certified copy

Documents must be current, and copies must be recently certified as a true copy of the original by a Justice of the Peace (JP) or a Commissioner for Declarations (Cdec). The certified copy must include their registration details, official stamp and date of certification.

Find your nearest JP or Cdec.

If you are overseas, you can get your documents certified by an Australian consulate or embassy official or a Notary Public.

Ensure that you have certified ALL supporting documents.

Processing time

Applications to correct personal information will generally be processed within seven business days.

Certificate issuing

A Senior Education Profile (SEP) is issued in the student’s legal name that is recorded by their main learning provider (MLP) in the student's QCAA learning account. A name correction or change can be requested if there is:

  • an incorrect name or spelling error recorded
  • a legal name change after the SEP was issued.

When the change of personal details has been processed:

  • Students issued their SEP in or after 2020 will have their documents available in their QCAA Student Portal.
  • Students issued their SEP prior to 2020 will need to request a replacement copy.

Note: A change of family name cannot be requested if the change is due to marriage.
