Correction of personal details

How to apply

Use this form to request a correction to your personal details recorded with QCAA:

  • All documentation must be stamped and certified by a Justice of the Peace or Commissioner of Declaration.
    The registration number and full name of the certifier must be included and be legible.
  • Documents not in English must be translated — see the Application notes: Translation.
  • Check information recorded is accurate before you submit your application.
