Terms and conditions for publishing an individual’s details for Certificate of Academic Commendation

In completing this form:

  • I understand I am a recipient of a Certificate of Academic Commendation award, and that this award is for students who achieve an A in at least six QCAA General subjects.
  • I agree to the QCAA publishing my name, school and subjects as a recipient of a Certificate of Academic Commendation at www.qcaa.qld.edu.au/certificate-academic-commendation.
  • I acknowledge the QCAA cannot prevent the unauthorised copying of this information from its website.
  • I acknowledge that I will not be paid, and my parent/guardian will not be paid for giving this permission or for the use of this information.
  • I acknowledge the QCAA uses its best endeavours to ensure the person signing this Permission Form is authorised to do so, but takes no responsibility for circumstances in which it is misled as to the identity or authority or ability of a person to provide permission.
  • I acknowledge that if the functions of the QCAA are transferred by law to another Queensland Government department or agency, this permission will apply to that department or agency.
  • I acknowledge that the QCAA will continue to publish the details of the Certificate of Academic Commendation award recipients at www.qcaa.qld.edu.au/certificate-academic-commendation.
  • I understand that if I wish to revoke this permission, I or my parent/guardian should send a notice in writing via email to communications@qcaa.qld.edu.au or by mail to:
