Kindergartens provide parents/carers a copy of their child's completed transition statement to review. Parents/carers may:
- make suggestions for continuity of learning
- request any incorrect personal information is amended.
A parent/carer may give permission for a completed and approved transition statement to be shared with their child’s Prep Year teacher and/or other relevant staff by signing a transition statement consent form (DOCX, 223.9 KB). Kindergarten services should retain a copy of the consent form with the transition statement for three years.
